Medication Guidelines

When an illness is serious enough to warrant the administration of medication, the parent of guardian should make every attempt to administer the medication before or after school hours.

If it becomes necessary for prescribed and/or over-the-counter medication to be given during school hours, a medication authorization form from the prescribing doctor or dentist shall be required. Additionally, the medication must be brought into school by a parent/guardian and must be labeled with the following information:

  1. Child's first and last name
  2. Child's grade
  3. Name of the medication
  4. Amount of the medication to be taken
  5. Time the child is to take the medication

The procedure is the same for controlled substance medications (i.e. Adderall, Concerta, Ritalin, etc.). All medication will need to be picked up at the end of the school year by a parent/guardian
